Neurological Symptoms Survey

The Neurological Normal & Abnormal Symptoms Survey is a civilian research and documentation survey created to help identify, compare, and better understand neurological, sensory, cognitive, and unexplained health symptoms reported by members of the public.

Many individuals experience occasional headaches, stress, fatigue, dizziness, ringing in the ears, or body discomfort as part of everyday life. Others report sudden, severe, unusual, or persistent symptoms that feel abnormal, unexplained, or unlike anything they have experienced before.

This survey is designed to help document both normal/common symptoms and abnormal/unexplained symptoms so that patterns can be reviewed, organized, and shared in a responsible way with leaders, lawmakers, researchers, medical professionals, and public health officials.

Purpose of the Survey

The purpose of this survey is to help:

Document civilian reports of sudden or unusual neurological and sensory symptoms.

Identify patterns involving head pressure, nerve pain, ringing in the ears, dizziness, cognitive changes, sleep disruption, external sounds, body sensations, or other unexplained symptoms.

Better understand how these experiences may affect daily life, work, sleep, decision-making, safety, and overall well-being.

Help inform elected leaders and public officials about what some of their constituents are experiencing.

Support future conversations about medical access, research, rehabilitation, public safety, and public policy.

This survey may be completed by individuals who are experiencing, or have experienced, symptoms such as:

Head pressure, headaches, migraines, or unusual head sensations
Ringing in the ears, buzzing, tones, or external sounds
Sudden nerve pain, burning, tingling, shocks, or body vibrations
Dizziness, balance issues, vertigo, or disorientation
Memory issues, brain fog, confusion, or difficulty concentrating
Sleep disruption, sudden fatigue, or unexplained physical changes
Abnormal sensory experiences or symptoms that feel new, unusual, or unexplained

You may also complete this survey if you are unsure whether your symptoms are normal, stress-related, medical, environmental, neurological, or unexplained.

This survey is not intended to diagnose, label, or evaluate participants for mental health conditions, including schizophrenia. Its purpose is to document reported experiences and support research into possible medical, neurological, sensory, environmental, and unexplained causes of sudden or abnormal symptoms.

This survey is also not a substitute for medical care. Individuals experiencing serious, sudden, or worsening symptoms should seek medical attention from a qualified healthcare provider.

Why Participation Matters

Every response helps build a clearer picture of what civilians are experiencing. By collecting information in one place, this survey may help identify patterns that are currently being overlooked, dismissed, or misunderstood.

The results may be used to help educate the public, inform elected leaders, support advocacy efforts, and encourage further research into neurological and unexplained symptom reports.

Individuals experiencing normal, abnormal, neurological, sensory, cognitive, or unexplained symptoms should be counted, documented, and included in conversations about research, medical access, support, and public policy.
