Purpose
This survey is designed for civilians who are experiencing unexplained neurological or sensory symptoms similar to those described in Anomalous Health Incidents (AHI), sometimes referred to as “Havana Syndrome.”
While much attention has focused on government personnel, civilians—including veterans and public service workers—have reported similar experiences, often without access to specialized evaluation, testing, or care.
This survey helps document these experiences, identify patterns, and support efforts to improve awareness, access to care, and informed research and policy discussions.
Participation is voluntary, confidential, and may be completed with or without a diagnosis.
